Understanding the WHO Integrated Care for Older People (ICOPE) framework that forms the clinical backbone of our platform.
The WHO Integrated Care for Older People (ICOPE) approach provides evidence-based guidance for community-level interventions to manage declines in intrinsic capacity. Published in 2019 and updated through 2024, the framework shifts the focus from disease-based models to person-centred care that maintains functional ability in older adults.
ICOPE defines a step-by-step pathway: screen for declines, conduct in-depth assessment of flagged domains, create a personalised care plan, and monitor progress over time. ICOPE Connect digitises this entire pathway into a seamless clinical workflow.
Each domain represents a key area of intrinsic capacity. Decline in any domain can impact a person's overall functional ability and quality of life.
Screened using standardised memory and orientation tests. ICOPE Connect supports the Mini-Mental State Examination (MMSE) with automated scoring and longitudinal tracking. Early detection of cognitive decline enables timely intervention and support planning.
Assessed through the Short Physical Performance Battery (SPPB) and Timed Up and Go (TUG) tests. These validated instruments measure balance, gait speed, and lower extremity strength. Results flag fall risk and guide physical activity recommendations.
Evaluated using the Mini Nutritional Assessment Short-Form (MNA-SF). This 6-item screening tool identifies older adults who are malnourished or at risk. ICOPE Connect tracks nutritional status over time and prompts dietary intervention when scores decline.
Screening involves standardised questions about difficulty hearing in conversation, using the telephone, or following group discussions. Flagged results prompt referral for audiometric assessment. Early intervention with hearing aids or assistive devices can significantly improve quality of life.
Screened with questions about difficulty reading, recognising faces, and navigating the environment. Patients reporting visual difficulty are flagged for ophthalmological referral. Tracking vision changes over time helps identify progressive conditions early.
Assessed using the Geriatric Depression Scale (GDS-15) or Patient Health Questionnaire (PHQ-9). These validated instruments detect depressive symptoms with high sensitivity. Flagged results trigger care plan recommendations including counselling referrals and activity programmes.
A structured, evidence-based approach from initial screening to ongoing monitoring.
Universal screening of older adults in community settings to detect declines in intrinsic capacity. ICOPE Connect enables patient self-screening from any device, removing barriers to early detection. Screening takes approximately 10-15 minutes.
In-depth clinical assessment of flagged domains using validated instruments. The clinician reviews detailed scoring, historical comparisons, and AI-generated trend summaries to understand the full picture of the patient's capacity.
Development of a multi-domain care plan tailored to the individual's priorities, preferences, and clinical needs. ICOPE Connect auto-generates care plan drafts from assessment data and supports AI-assisted intervention recommendations.
Ongoing monitoring with periodic re-screening to track changes in intrinsic capacity. ICOPE Connect's longitudinal trend charts visualise progress across all domains, enabling clinicians to adjust care plans based on real trajectory data.

WHO ICOPE ResourcesIntegrate ICOPE Connect into your existing health systems with our RESTful API. Available on Enterprise plans.
All API requests require JWT bearer token authentication. Tokens are issued via the /auth/login endpoint with email-based 2FA verification.
Standard HTTP methods (GET, POST, PUT, DELETE) with JSON request/response bodies. Consistent error handling with structured error objects.
API requests are rate limited per endpoint category. AI-powered endpoints have separate rate limits to manage compute costs. Limits are returned in response headers.
All API access uses JWT bearer tokens with short-lived access and refresh token rotation.
Authenticate with email and password. Returns a short-lived access token (30 min) and a refresh token (7 days). If 2FA is required, the response includes a requires_otp: true flag.
Submit the 6-digit OTP code sent to the user's email. On success, returns the full token pair. Optionally include trust_device: true to skip 2FA for 30 days on this device.
Exchange a valid refresh token for a new access/refresh token pair. Old refresh tokens are invalidated on use (token rotation). Returns 401 if the refresh token is expired or revoked.
Include the access token in all subsequent requests: Authorization: Bearer <access_token>. Role-based access control is enforced at every endpoint.
Primary resources available through the API. All endpoints require authentication and appropriate role permissions.

ICOPE Connect was created to bridge the gap between the WHO's evidence-based ICOPE framework and the reality of frontline clinical practice. We believe that every older adult deserves access to person-centred, technology-enhanced care.
The world's population is ageing rapidly. By 2030, 1 in 6 people globally will be aged 60 or over. The WHO developed the ICOPE framework to address this, but adoption has been slow due to the complexity of implementing multi-domain screening at scale using paper-based tools and fragmented systems.
Clinicians recognised the value of ICOPE but lacked a unified digital tool to screen across all six domains, generate care plans, and track patients over time. Paper-based workflows are error-prone, time-consuming, and make longitudinal analysis nearly impossible.
ICOPE Connect digitises the complete ICOPE care pathway into a single, modern platform. From patient self-screening on any device to AI-assisted clinical decision support, we handle the technology so clinicians can focus on what matters: their patients.
We envision a world where every older adult has access to standardised, person-centred care regardless of where they live. By combining the WHO ICOPE evidence base with modern AI and cloud technology, we aim to make comprehensive geriatric care accessible globally.
Patient data security is non-negotiable. Every design decision starts with privacy.
Built on WHO-validated instruments and guidelines. No shortcuts with clinical rigour.
Technology serves the human relationship between clinician and patient, never replaces it.
Multi-language, mobile-first, and designed for diverse healthcare settings worldwide.
A robust technology stack designed for reliability, scalability, and security in clinical environments.

Health Information: Screening responses, assessment scores, care plan data, and longitudinal health records entered through the platform. This information is collected to provide clinical decision support and enable the ICOPE care pathway.
Usage Data: We collect anonymised usage data including pages visited, features used, and session duration to improve the platform. We do not use tracking cookies for advertising purposes.
Device Information: When you trust a device for two-factor authentication, we store a device fingerprint (browser type, operating system) to recognise your device for up to 30 days. This data is used solely for security purposes.
We use your information exclusively for the following purposes:
We implement comprehensive security measures to protect your data:
Your data is stored on Microsoft Fabric SQL with cloud infrastructure provided by Amazon Web Services (AWS). Health records are retained for the duration of your active account and for a minimum retention period as required by applicable healthcare regulations in your jurisdiction.
Account data is retained for 90 days after account deactivation to allow for reactivation. After this period, personal data is permanently deleted unless retention is required by law. Anonymised, aggregated data may be retained indefinitely for platform improvement and research purposes.
The AI Clinical Assistant is powered by Azure OpenAI (GPT-5). When you use AI features, relevant patient assessment data is sent to Azure OpenAI for processing. Microsoft's Azure OpenAI Service operates under enterprise data protection agreements: your data is not used to train AI models, is not accessible to other customers, and is processed in compliance with Microsoft's data processing terms.
We use AWS Simple Email Service (SES) for email notifications and AWS Simple Notification Service (SNS) for SMS notifications. These services process only the minimum data required to deliver messages (email address or phone number and message content).
